Medical Records Request
Penn Highlands Clearfield Health Information Management (Medical records) develops and maintains a comprehensive medical records system. After your discharge, you can obtain copies of your medical records. Effective January 1, 2014, release of medical records for personal use will be charged according to the guidelines of the United States Department of Health and Human Services and the Pennsylvania Department of Health.
NOTE: Release of medical records for continuing medical care and treatment is free.

We charge a small fee for medical records on a per page basis. Fees apply for both electronic and paper copies. There are also flat fees for records to support Social Security claims, financial aid programs, and district attorney requests. Charges may also be assessed for the actual cost of postage, shipping and delivery of the requested records. Please call (814) 768-2370 for the latest rates.
NOTE: Federal regulations enacted under the Health Insurance Portability and Accountability Act at 45 CFR Parts 160—164 state that covered entities may charge a reasonable cost based fee that includes only the cost of copying, postage and summarizing the information (if the individual has agreed to receive a summary) when providing individuals access to their medical records. The Department of Health and Human Services has stated that the fees may not include costs associated with searching for and retrieving the requested information.
