Home Health Supervisor

Position Summary: The Home Health Supervisor, under supervision of the Home Health/Hospice Director, coordinates patient care, assures quality, and directs team activity in collaboration with other health care team members.

Responsibilities

*Complies with PENN HIGHLANDS DUBOIS policies and procedures, accreditation agency requirements, and federal, state or local law and regulations

*Uses the nursing process in the delivery of care as evidenced by but not limited to:

  • Obtains patient history and conducts appropriate patient assessment
  • Collaborates with the health care team to develop and manage the plan of care, involving patient/significant other whenever possible
  • Implements appropriate nursing care to meet patient needs
  • Evaluates patient's response to medical, nursing, and other disciplines’ interventions and takes appropriate action

*Demonstrates critical thinking and clinical competency in the delivery of patient care as evidenced by but not limited to:

  • Administers medications, monitors effectiveness, and takes appropriate action as needed in accordance with policies and procedures
  • Proficient in starting and managing IV administration
  • Responds appropriately to changes in patient condition
  • Consults medical record for pertinent patient information such as diagnostic test results, physician progress notes
  • Implements the plan of care according to physician’s, nursing orders, clinical pathways, and nursing care standards
  • Appropriately delegates to and manages patient care delivered by support staff

*Manages patient teaching and discharge planning as evidenced by but not limited to:

  • Identifies patient learning needs on admission and throughout the episode of care
  • Collaborates with health care team to develop and implement the discharge plan
  • Prepares patient and significant others for appropriate home management regarding medications and the follow up plan of care

*Participates in the development of new staff as evidenced by but not limited to:

  • Actively participates in the orientation including cultural and social integration of new staff
  • When serving as preceptor, collaborates with staff member and manager/charge nurse to manage the learning process and develop individualized action plans as needs are identified

*Demonstrates professional accountability as evidenced by but not limited to:

  • Achieves educational credits to meet hospital and/or licensing/certification requirements
  • Presents information obtained at education conferences to staff members
  • Participates in the development and advancement of departmental standards, policies, and other clinical/professional initiatives
  • Conducts self in professional manner in dress, actions, and interpersonal communication
  • Assures environment of safety for patients, significant others, and coworkers
  • Informs Director/Supervisor/Clinical Manager of patient care situations and departmental problems as appropriate
  • Demonstrates effective problem solving skills for clinical and departmental issues

Unit Specific Duties

*Demonstrates and maintains ongoing competencies for care of medical/surgical and telemetry patients

*Maintains discipline specific basic arrhythmia competency

*Maintains current CPR and ACLS certifications

*General duties as directed

Qualifications

  • A Registered Nurse with a valid PA Nursing License.

  • One to three years of Home Health and or Hospice experience preferred.

  • Prior management experience preferred.

  • Working knowledge of OASIS and Home Health Regulations.

  • Ability to work independently and within a multidisciplinary team.

  • Valid drivers’ license and proof of valid insurance.

Director, Physician Network Huntingdon/Tyrone Region


The Director assists in the planning, organizing, and providing Physician Network Services to the Medical Center's Physician Practices, subsidiaries, and other related interests. Coordinates the practice supervision activities of clinical and non-clinical staff; oversees operations of assigned practices. Acts as a liaison for the practice staff with operational oversight to the department. Supervises the various clinic staff positions.Â

Job Qualifications

  • High School Degree with 5 years management experience required. Bachelors degree in Business Administration or related Health Care/ Business field or completion of bachelor degree within five years of start date in director role required.
  • At least 5 years experience in physician office or related experience. Minimum of 2 years experience in supervisory role.
  • Knowledge of physician office procedures required
  • Must have a wide range of billing, charging, and reimbursement guidelines experience
  • Healthcare Provider Certification at hire
  • MGMA membership and certification, and coding certification preferred.Â

Operating Room Technician

Penn Highlands Huntingdon has an immediate need for a Part-time Operating Room Technician

The individual in this position is responsible and accountable for sorting, assembling, and handing surgical instruments and supplies throughout the perioperative period.


Minimum Required Qualifications:

Licensed Practical Nurse

Penn Highlands Huntingdon is in immediate need for Licensed Practical Nurses in the AC1 Unit. A nurse working in the Unit is responsible for assisting RN in assessing, planning, providing, and evaluating age-specific patient care.

Requirements include:

  • A current licensure in the State of Pennsylvania.
  • A minimum of one year experience.
  • ACLS, BLS

Environmental Services Aide

Penn Highlands Huntingdon has an immediate need for an Environmental Services Aide

The individual in this position would be responsible to maintain a clean, safe and controlled environment with respect to infection control and quality control

Food and Nutrition Aide

Penn Highlands Huntingdon has an immediate need for a Part-time Food Services Aide.

The individual in this position will assist with preparing and serving food trays to patients and food to other customers served by the Hospital.

This individual will also assist in cleaning up after meals.

This individual will also help at special functions hosted by the Hospital.

Minimum Required Qualifications:

  • High School Diploma or Equivalent
  • 18 Years of age or older

Registration Clerk

Penn Highlands Huntingdon has an immediate need for a Registration Clerk in the Emergency Department.

The individual in this position would be responsible registering Outpatient, Inpatient and/or Emergency Room Patients.

The individual must be able to effectively communicate with patients and their families as well as physicians and co-workers.

Minimum Required Qualifications:

  • Graduate of an approved Medical/Secretarial
  • Six (6) months previous experience in clerical or switchboard will be considered in lieu of degree

Physical Therapist

Penn Highlands Huntingdon has an immediate need for a PRN Physical Therapist

The individual in this position will use Physical Therapy Assessment and Evaluation techniques to determine the patient's assets and limitations.

The individual will establish goals and plan-aged appropriate treatment in collaboration with the patient/family and treatment team.

Minimum Required Qualifications:

  • Licensed as a Physical Therapist in PA.

Speech Therapist

Penn Highlands Huntingdon has an immediate need for a PRN (as needed) Speech Therapist.

The Candidate is responsible for providing diagnostic and therapeutic speech, language and swallowing services.

The candidate would provide service in the outpatient therapy department and also provide inpatient services.

Must have a current PA Speech/Language Pathology license.

Registered Nurse (Cardiac Cath Lab)

Penn Highlands Huntingdon has an immediate need for a Full-Time, Rotating Cardiac Catheterization Lab Nurse. This position performs the primary functions of a Registered Nurse in assessing, planning, implementing, and evaluating the care of all patients in the Cardiac Catheterization Lab during his or her shift. This position will perform scrub and hemodynamic monitoring duties with diagnostic and interventional cardiac procedures, and peripheral vascular procedures using clinical knowledge, technical skills, and professional judgment in accordance with professional and departmental standards and protocols.

Minimum requirements include:

  • Graduate from an accredited school of nursing
  • Current licensure in the state of Pennsylvania
  • Specific orientation to the Cardiac Catheterization Lab and Same Day Surgery Unit
  • Available to fulfill "on-call" obligations - response less than 30 minutes as requested
  • Demonstrates clinical competence in basic cardiac physiology/pathology, 12-Lead EKG interpretation; physiologic monitoring (intra cardiac pressure waveform knowledge), pacemaker operations and equipment related to cardiac care.

  • 2 years nursing experience required with diagnostic and interventional cardiac catheterization procedures.

Registered Nurse (Behavioral Health Department)

Penn Highlands Huntingdon is seeking (experience preferred) Registered Nurses in the BHU department. All positions Rotating. Current ACLS and PALS are preferred. Must be a graduate of an approved Diploma, associate or bachelor degree program in nursing and eligible to take the State Board Examination for licensure as an RN in Pennsylvania. Registered Nurse ED (Emergency Department)

Penn Highlands Huntingdon is in immediate need for Registered Nurses in the Emergency Department. A professional nurse working in the ED is responsible for assessing, planning, providing, and evaluating age-specific patient care in an emergency setting based on the PA Nurse Practice Act and knowledge acquired through education programs and learning experiences.

Requirements include:

Certified Registered Nurse Practitioner/Physician Assistant (Oncology)

Penn Highlands Huntingdon has an opening for a CRNP/PA in the Oncology Unit. The oncology unit is scheduled to open October 1, 2020. The Advanced Practice Provider functions as an essential member of the team in the practice setting and on units as indicated by physician needs.

Responsibilities

*Complies with PENN HIGHLANDS DUBOIS polices and procedures, accreditation agency requirements, and federal, state or local law and regulations.

  • Makes decisions regarding data gathering and appropriate management and the treatment patients being seen for the initial evaluation of a problem or the follow-up evaluation of a previously diagnosed and stabilized condition.
  • Initiate, by the observation of patients condition, appropriate evaluation and care provided within the practice setting and as directed within the hospital setting.
  • Examines patient, performs comprehensive physical examination, and compiles patient medical data, including health history and results of physical examination
  • Establish medical priorities based on the assessment findings.
  • Administers or orders diagnostic tests, such as x-ray, electrocardiogram, and laboratory tests, and interprets test results for deviations from normal
  • Performs therapeutic procedures as delineated on the clinical privileges form.
  • Develops and implements patient management plans, records progress notes, and assists in provision of continuity of care
  • Provides patient care and instructions as delegated by the physician supervisor that are within the scope of the physician supervisors practice, and the physician assistant is qualified to perform.
  • Performs clinical procedures as delineated in the written agreement between the supervising physician and the physician assistant
  • Performs clinical procedures and takes orders to dispense drugs as reviewed with the supervising physician.
  • Reviews patient treatment regimen and changes the regimen and provides this information to the treating physician.
  • Coordinates patient care with other treatment members.
  • Counsels and instructs patients regarding their health care.
  • Assures adequate patient health records are maintained and transferred as required when patients are referred to other facilities.
  • Attends and reports at all appropriate meetings.
  • Actively teaches patients, when appropriate, and provides staff training to promote effective patient education.
  • Provides instruction and professional educational programs for staff.
  • Performs administrative duties as assigned.
  • Promotes scientific inquiry in clinical practice using the research process to improve practice.
  • Supports the data collection, analysis, and evaluation required for performance improvement initiatives.
  • Applies statistical and analytic knowledge in performance improvement activities.
  • Identifies PI initiatives and supports the required data collection, analysis, and evaluation required for performance improvement initiatives.
  • Oversees the development and implementation of clinical pathways and nursing care standards specific to the needs of the Orthopaedic patient, with other team members.
  • Conducts community educational programs to support the business development.
  • Provides on call coverage under the supervision of the physician as requested.
  • Other duties as directed.

Qualifications

  • Masters Degree in Nursing required.
  • Current Pennsylvania Registered Nurse licensure required.
  • Current certification as a nurse practitioner from a board-recognized national certification organization (as per the Pennsylvania state board of nursing) which requires the passing of a national certifying examination in the particular clinical specialty area in which the applicant is seeking certification by the board required.
  • CPR certification from the American Heart Association required.
  • Minimum of 1 year experience as a Nurse Practitioner preferred.
  • Maintains current ACLS and PALS certifications preferred. OR
  • Completion of an accredited Physician Assistant program required.
  • Maintain a license in the Commonwealth of Pennsylvania required.
  • National certification required.
  • CPR certification from the American Heart Association.
  • Minimum of 1 year experience as a Physician Assistant preferred.
  • Maintains ACLS and PALS certifications preferred.
