Welcome Michelle Jacobson, CRNP

Michelle Jacobson, certified registered nurse practitioner, is now seeing patients alongside Dr. Robert Usaitis and Dr. Kara Kozloski at Penn Highlands Family Medicine, located at 145 Hospital Avenue, Suite 205, DuBois.

Jacobson, who is board certified by the American Academy of Nurse Practitioners, provides wellness exams and vaccinations as well as treatment and management for both acute and chronic illnesses.

“Michelle comes to us with a wealth of knowledge and experience through her 17 years of employment at Penn Highlands,” said Usaitis. “She is capable of evaluating and treating all patient problems and is a welcome addition to our staff.”

“Michelle is going to be a wonderful addition to our practice,” added Kozloski. “I believe she is going to make our practice even better and more efficient in caring for our patients.”

“I was born and raised in the area,” said Jacobson. “I am pleased to continue my career at Penn Highlands as I transition into my new role from registered nurse to family nurse practitioner. I am honored to join such a great group of staff at Penn Highlands Family Medicine.”

Jacobson earned her associate degree from Lock Haven University in Lock Haven and her bachelor’s degree from Pennsylvania State University in State College. She then earned her master’s degree in nursing at Clarion University in Clarion and Edinboro University in Edinboro.

She is a member of the American Association of Nurse Practitioners, the American Association of Critical Care Nurses and the Pennsylvania Coalition of Nurse Practitioners.

Jacobson is a native of Punxsutawney and enjoys hiking, reading and spending her free time with her family.
