If you or a loved one could benefit from a helping hand at home because of a health issue, Penn Highlands Community Nurses offers a range of home health care services to meet your needs. Whether you require in-home rehabilitation therapy after surgery, assistance with activities of daily living such as personal grooming or getting in and out of bed, or palliative care if you’re dealing with an advanced illness, they can help.

What Types of Home Health Care Services are Provided?

After a hospital stay, you may be well enough to go home, but you may not have the help or ability for your more complicated medical care. Or perhaps you have a chronic condition or advanced illness that will need constant monitoring, and you need assistance. Even if you have family and friends to serve as caregivers, sometimes things can get overwhelming. Our wide range of home health services can help you get the care you need and stay in the comfort of your own home. Penn Highlands Community Nurses offers the following services:

Palliative Care
Those with advanced illnesses can face unique challenges, like frequent trips to the hospital or doctors’ offices for the management of pain and other symptoms. Our supportive care staff offers you and your and family education, pain and symptom management, care coordination, and other services.

Private Duty Home Support Services
If you’re elderly, disabled, or simply need extra care at home after an illness or surgery, our private duty home health aides and nurses can help you remain independent and safe in your home. We assist on an hourly basis with personal care (bathing, dressing, assisting with moving from bed to chair), home management services (preparing and serving meals, light housework, laundry), and respite care. If needed, we also provide private duty skilled nursing services such as medication administration, nutrition counseling, injections, catheter care, urine collection, and dressing changes.

Rehabilitation Services
If you’re recovering from surgery or working to regain mobility after an injury or illness, you could benefit from our in-home rehabilitation services. Physical, occupational, and speech therapists assist with strength and range of motion exercises as well as other rehab protocols, such as massage, icing, and taping.

Specialty Home Care Programs
For home health care needs that require specialized knowledge and training, we offer Specialty Home Care Programs for those with chronic obstructive pulmonary disease (COPD) Program, congestive heart failure (CHF), and diabetes. We also provide home infusion therapy, wound and ostomy care, and skilled nursing services for expectant mothers as well as newborns who need a little extra TLC once they arrive home.

Bereavement Support Services
Our grief counselors offer guidance, education, and compassion for those who have experienced the loss of a loved one. Services include counseling and support services.

Medical Social Services
We can assist you with the social, emotional, and financial issues you may encounter when you need home health care services.

Who would Benefit from In-home Care vs. Care at a Facility?

Home health care services are available for patients who no longer need to be in the hospital but still need professional care, teaching, and assessment. For them, health care comes to their home through visits by Penn Highlands Community Nurses staff.

Many home health patients have chronic diseases, such as heart disease, high blood pressure, chronic obstructive pulmonary disease (COPD), diabetes, or lung disease, that require ongoing changes in their plan of care. Others may need short-term care with wound care or help with getting medications or nutritional therapy delivered by infusion.

Patients may need daily care, one visit a month or any variation in between. It depends on the diagnosis, but on the average, most patients are visited about one or two times per week.

What Types of Health Professionals Provide Home Health Care Services?

At Penn Highlands Community Nurses, there is a team comprised of registered nurses, personal care aides, physical therapists, occupational therapists, speech therapists, social workers, and nutritionists. We evaluate each patient and work with referring physicians to determine the best team to care for your needs.

Home Health Care Costs

The cost of home health care and whether Medicare or other insurance plans will cover it depends on your condition and the services you receive. Penn Highlands Community Nurses provides a wide range of both home care and home health care services. In general home care provides help with activities of daily living such as bathing, housekeeping, and transportation.

Home Health care offers medical care such as checking a patient’s vital signs, assisting with medication, and providing physical therapy or infusion.

In general, home care is not covered by Medicare or insurance. However, home health care—particularly if it is part of your recovery from a hospital stay—is generally covered. Our staff will help you understand your costs and talk with you about payment options. Call us at 800-296-4065.
